ForeseeHome Physician FAQs 

Q.  How does a patient subscribe to the ForeseeHome AMD Monitoring Program?

A.  Patients first must obtain a comprehensive eye exam and prescription from a doctor at a certified ForeseeHome Referral Center. 

 

Q.  How can I start prescribing the ForeseeHome to my patients?

A.  Notal Vision is currently conducting a limited, pre-launch marketing trial.  This select group of practices has completed the enrollment and training process required to become a certified ForeseeHome Referral Center.  If you would like to become a certified ForeseeHome Referral Center, please contact us. 

 

Q.  What patients are ForeseeHome candidates?

A.  Any patient with intermediate AMD is a candidate.  Patients must be diagnosed with dry age-related macular degeneration.  Patients must also have visual acuity of 20/60 or better and must have the cognitive and physical ability to use the ForeseeHome device.  Patients must be able to establish a baseline on the ForeseeHome device.

 

The ideal candidate is the patient who already has neovascular AMD in one eye.  These patients are at high risk of developing wet AMD in their fellow eye.

 

Q.  How is my practice informed of changes in patient results?

A.  The Notal Vision Data Monitoring Center (DMC) consists of a team of experts who analyze patient results and provide alerts and information to the referral center practice.  Each patient’s results are compared to a normative database and the patient’s personal threshold.  When a patient’s results indicate a statistically significant change, an alert will trigger.  The DMC will contact (email and/or phone) the pre-assigned contact person at the practice to inform them of the patient alert.  The DMC will first notify the practice and then will contact the patient.  Aditionally, the DMC will follow up with the practice to ensure the patient is scheduled promptly for a comprehensive exam.

 

Q.  How can I see my patient’s results?

A.  Certified ForeseeHome Referral Centers can view their patient results at any time through our secure online database.

 

Q. How does my patient obtain the ForeseeHome device?

A.  Upon receiving a prescription from a ForeseeHome Referral Center, Notal Vision will ship the ForeseeHome device directly to the patient’s home.  Patients are instructed to contact Notal Vision when setting up their device at home the first time.  Notal Vision customer service will provide live assistance during the set up process and is available for any other questions related to use of the ForeseeHome.

 

Q.  How can my practice become a certified ForeseeHome Referral Center?

A.  Practices interested in becoming a certified ForeseeHome Referral Center must first complete an enrollment agreement.  Notal Vision will then conduct a training session in your practice, teaching appropriate staff and doctors about the ForeseeHome AMD Monitor and Program.  We’ve found it works best when the practice can schedule 5-10 patient candidates for the training session.  Training usually takes about an hour and patient candidates can be scheduled every 30 minutes thereafter on the training day.

 

Q.  How does Notal Vision support my practice as a Referral Center?

A.  The Notal Vision customer service line is easily accessible to patients with questions about billing, usage and other support issues. Additionally, Notal Vision will contact patients if no tests are received for 7 days in a row to troubleshoot any problems they may be having and to stress the importance of daily testing.
